The short version
Sticks of rolled paper, plastic or wood are dropped into clips on a moving conveyor. Adhesive goes on the ends, a measured wad of cotton from a coil is attached, and a shaper moving at a different speed from the conveyor rolls the wad into a firm tip. High-speed lines then count the finished swabs into boxes.
-
Make the sticks
Plastic stems are injection molded or extruded in thin rods from polypropylene or polyethylene. Paper stems are rolled or wrapped into a rigid tube, a more common choice since bans on plastic stems began.

Prepare the cotton
After ginning and carding to align the fibers, the cotton arrives at the factory as loose fiber or a coil of cotton wool.
Goes in -
Load the conveyor
Individual sticks are deposited on a continuously moving belt or chain, where clips hold each one.
-
Glue the ends
An applicator puts adhesive on one or both ends of each stick.
Goes in -
Attach and shape the tip
A portion of cotton coil sticks to the glued end. A shaper bar or belt, moving at a different speed than the conveyor, rolls it against its surface and forms a tight tip under tension.
-
Check and box
Automated lines look over the tips, then count and pack them. Double-tipped swabs get the same treatment at each end in sequence.

Worth repeating
- Leo Gerstenzang developed the first mass-produced swab in 1923 after watching his wife wrap cotton on toothpicks.
- The earliest name was Baby Gays, changed to Q-Tips Baby Gays in 1926, where Q stands for quality.
- A wooden handle and a paper sleeve let a medical swab be sterilized in an autoclave.
