Cough syrup is a sugar solution thick enough to resist crystals, carrying a small measured dose of medicine.

The short version

A syrup plant links a sugar tank, a manufacturing tank and a storage tank by pipeline. Sugar dissolves in purified water first, then the medicine, stabilizers, flavor and color are mixed in, usually on a tank sitting on load cells so the batch is made up by weight. The liquid is filtered, sampled and piped to a bottle filler.

  1. Weigh out the materials

Every ingredient is dispensed and weighed against the bill of materials, then moved to the production area.

  2. Dissolve the sugar

Sugar goes into purified water in a dedicated tank, heated until the liquid turns clear and viscous. Heat-sensitive products skip the heat and rely on agitation.

    Goes in
  3. Stop it crystallizing

Stabilizers such as glycerin keep the sucrose from forming crystals, and surfactants help the other ingredients dissolve clear.

    Goes in
  4. Blend on load cells

Syrup moves by pipe to the manufacturing tank, where the medicine, flavor and color join it. Common actives are guaifenesin and dextromethorphan. The FDA notes tanks usually sit on load cells, since dipsticks and tank markings have proved inaccurate.

  5. Filter out precipitate

Filtration catches anything that has dropped out of solution, and the clear liquid goes to a storage tank.

  6. Sample the bulk tank

Most makers assay samples from the bulk before filling. Inspectors also look for microbes, since a contaminant points to a weak process or an inadequate preservative.

  7. Fill the bottles

Storage tank outlets feed a filling machine that doses pre-cleaned bottles. Long delivery lines can harbor contamination, so they have to be cleanable. Measuring devices packed with the bottle must be calibrated.

  8. Carton and box

Bottles go into cartons and then corrugated boxes, to be kept cool, dry and dark.

Worth repeating

  • Cough medicines have been used by 10 percent of American children in any given week, though they are not recommended for children six or younger in Canada or the United States.
  • In 2015 the FDA warned about codeine-containing cough medication in children.
